What is Home Care Package?

Home Care Packages (HCP) are the federal government’s way of funding services for people who have additional care needs and wish to continue living independently.

They’re designed to be flexible and to provide support across a range of areas, including:

  • Medication administration
  • Transport
  • Taking part in social activities

Home Care Packages can also include nursing, podiatry and allied health services.

Am I eligible for a Home Care Package?

To qualify, you must be 65 or older (50 years or older for Aboriginal and/or Torres Strait Islander peoples) and need help with everyday tasks.

Eligibility is assessed by My Aged Care, and is based on your health and well-being, mobility, independence and personal support networks

How does U and V care provide Home Care Package support?

U and V care partners with Trilogy Care to manage and support your HCP package.

Our Client Services Manager will help onboard you to U and V care and match you with the right support team member.

Once you have attended a Care Team Meeting arranged by Trilogy Care, U and V Care will assist you in managing all the support you need and ensure that your HCP meets your requirements.

Step 1
  • Call My Aged Care on 1800 200 422 and press option 5
  • Reactivate your Home Care Package referral code

You can also do this on My Aged Care online portal.

Step 2
  • Call us at 03 8313 0656 or Mobile 0403 464 854
  • Provide your referral code
  • Begin onboarding
  • We can arrange to this over the phone, online or in your home
Step 3
  • Provide notice to your current provider that you intend to transfer
  • Nominate a termination date (try to give your current provider 2 weeks notice)
